Permission Slips

Permission Slips -- Every event that takes place outside of the church (or extended ones w/in the church) requires a Permission Slip! Don't forget!! Extra copies are also stored in the information center.

Health Form -- Always update your Health Form when health conditions,  insurance policies,  addresses, or phone numbers change. We keep Health Forms with us during all youth functions in case there would be a need for medical treatment.
